HEALTH CARING CARDS

Physicians report that a lack of medical history by many patients, in particular non-English speaking patients, is one of the most important issues they face. The Health Caring Cards provide space for health history along with a list of easy to understand questions suggested by experienced physicians for SAVE THE PATIENT as being most likely to enhance a patient's understanding of any medical encounter. The cards are in English on the front of the card, and in either Spanish, Polish, or Chinese on the reverse. We plan to add additional languages, including Braille, as donations and funding allow.

Health Caring Cards are wallet-sized cards in plastic sleeves which fit easily into a wallet or purse. They address five subject areas for use when visiting doctors, surgeons, hospitals, and prescription drug providers: your health history; what you should do before your visit; questions to ask during your visit; questions about medications; and patients rights.

The cards are free to the public.
